We need some information to grant you a MyChart account. After you submit this request, it may take up to a week for processing and verification. When you're approved, you will receive an email or a letter with your activation code and instructions on how to activate your MyChart account. Ready to get started? We just need some information and you’ll be on your way. If you have an existing Advocate LiveWell account, your username and password will also allow you to access PediaTrust MyChart and you do not need to set up a new account.
If you are over the age of 18, fill in the fields below. We’ll ask you a few questions via a third-party verification system for security reasons. Once verified, we’ll send you an email or text with your activation code and instructions to activate your MyChart account. Need access to your child? Log in and select Request Access to Your Child’s Record.
Have a MyChart account but experiencing trouble signing in?
• Locked out of your account – Contact the support team at 224-338-5437 (KIDS) or mychart@pediatrust.com
• Forgot your username or password – Use the Forgot Username? or Forgot Password? links beneath the Sign In button.
• Can’t access your child’s account – Log in and select Request Access to Your Child’s Record. Access should be available within five business days.
